Body
...Fiscal Impact/Change to Net County Cost:
No change to Net County Cost. Risk Management estimates this amendment will increase the contract by approximately $75,000 to a total contract amount of approximately $450,000.  Charges are based on actual claims plus an administrative fee of $1.40 Per Eligible Enrollee. The claims and administrative fees are budgeted within Risk Management's internal service fund, health benefits sub-fund. Revenue is provided by Board approved internal charges along with charges to retirees and outside agencies on behalf of participants in the County's self funded vision benefits program.

...Background and Reason for Recommendation:
Vision Service Plan services includes claims administration and a network of Preferred Member Doctors to provide vision services to County employees. This agreement is retroactive to July 1, 2011 due to delays by the vendor in submitting the contract amendment document.

...Action to be taken following Board approval:
Following Board approval: Chairman will sign the amendment document
