Title: Health and Human Services Agency, Community Services Division, recommending the Board adopt Resolution 125-2012 authorizing the Chair to sign the amended Medi-Cal Targeted Case Management Provider Participation Agreement 09-0713A2 with the California Department of Health Care Services, extending the term from July 1, 2012, to June 30, 2013 for Claiming Federal Financial Participation and reimbursement for Targeted Case Management (TCM) Services. The resolution further authorizes the Agency Director, or successor, or the Chief Assistant Director, or successor, to continue to administer Agreement 09-0713A2, as amended and execute subsequent documents relating to said Agreement, including amendments thereto, contingent upon County Counsel and Risk Management approval, that do not affect the dollar amount or the term, and any subsequent required fiscal and programmatic reports. Fiscal Impact/Change to Net County Cost
There is no specific dollar amount attached to this Provider Agreement as reimbursable services are provided based on need.  Approximately $144,000 in Targeted Case Management (TCM) revenues is included in the Agency's Fiscal Year 2012/13 Recommended Budget.
Background
On May 22, 2007 the Board of Supervisors approved and authorized execution of Medi-Cal Targeted Case Management Provider Participation Agreement 09-0712 and the accompanying CCC307 Certification for claiming Federal Financial Participation and reimbursement for TCM services during the term July 1, 2007 through June 30, 2012.    
 
Reason for Recommendation
Medi-Cal Targeted Case Management Provider Participation Agreement 09-713A2 amends Agreement 09-0712 to extend the termfrom July 1, 2012, to June 30, 2013, reflect a reorganization and name change from the State Department of Health Services to the State Department of Health Care Services, and requires compliance with the California Medicaid State Plan which redefined target populations effective July 1, 2011.
Currently only the Public Guardian Office provides Targeted Case Management (TCM) Services to Medi-Cal clients although other County departments and certain community agencies have participated in the past.  The TCM Reimbursement Program is federally funded and regulated with the objective to assist eligible individuals in gaining access to needed medical, social, educational and other services.  The State Department of Health Care Services compensates El Dorado County based on reported and documented "encounters", payable at the end of each fiscal year.  The Health and Human Services Agency acts as County liaison for receipt of TCM reimbursements and El Dorado County has been participating in the TCM Reimbursement Program since 1998.
Retroactive Term
Upon execution, Agreement 09-713A2 is retroactive to July 1, 2012.  The Agreement was received from the Department of Health Care Services on July 23, 2012 and processing was expedited.
Action(s) to be taken following Board approval
1)      Clerk of the Board to provide Health & Human Services at Briw Road, with two (2) copies of Agreement 09-0713A2 and two (2) certified resolutions signed by the Chair.
2)      Health and Human Services to distribute as appropriate and return one fully executed Agreement to the Board.
