Title
Health and Human Services Agency, Public Health Division, recommending the Board consider the following:
1) Authorize the Chair to sign retroactive Amendment II to Agreement for Services 024-S1010 with Marshall Medical Center for the provision of “as requested” laboratory services which will extend the term to May 31, 2015 and increase the maximum contractual obligation to $100,000 for the term of the Agreement; and
2) Authorize the Chief Administrative Officer or their designee to execute further documents relating to Agreement for Services 024-S1010, including amendments thereto, contingent upon approval by County Counsel and Risk Management, which do not affect the dollar amount or term of the Agreement.
FUNDING: Public Health Realignment.

There is no Net County Cost associated with this Agreement. Sufficient appropriations were included in the Fiscal Year (“FY”) FY 2013-14 Budget Request, and will be included in the FY 2014-15 Budget Request.
Background
The Health and Human Services Agency ("HHSA") has contracted with Marshall Medical Center (“Marshall”) for the provision of miscellaneous laboratory, pathology, and x-ray diagnostic services on an “as requested” basis since 2005. Agreement for Services 024-S1010 with Marshall for these services was executed by the County Purchasing Agent on June 28, 2010 and its Amendment I was also executed by the Purchasing Agent on October 29, 2012.
