Chief Administrative Office, Procurement and Contracts Division on behalf of Risk Management, recommending the Board approve and authorize the Chair to sign Amendment VI to Agreement No. 288-S1111 with Occu-Med, LTD, increasing the compensation by $25,000 to process final invoices for the Agreement, for a total not to exceed amount of $535,000, contingent upon approval by County Counsel and Risk Management. This Agreement provided for occupational health services, specifically in the areas of pre-employment (post-offer) physical evaluations, immunizations and fitness for duty examinations for the Human Resources Department through June 30, 2014.

Administrative costs are budgeted and paid through the Risk Management Liability Fund and the remainder is absorbed by departments as part of their pre-employment costs for the period of the contract.
Background
The County of El Dorado has been using Occu-Med, LTD, to coordinate pre-employment, reasonable suspicion, and fitness-for-duty medical exams since the Board approved the original Agreement on November 15, 2010. This contract was previously amended on November 15, 2011, July 17, 2012, October 30, 2012, and April 9, 2013 to increase the total not to exceed amounts and terms of the contract. Amendment V was for a three (3) month term extension only while Risk Management completed a Request for Proposal (RFP) which was awarded to a new vendor, The Permanente Medical Group, on June 24, 2014. Amendment VI to this Agreement is for an increase in compensation only to process the final invoices for the Agreement.
Reason for Recommendation
An increase in compensation is necessary to process the final invoices for this Agreement.
Action(s) to be taken following Board Approval
Following Board approval, Procurement and Contracts will finalize Amendment VI to Agreement No. 288-S1111 with Occu-Med, LTD, contingent upon approval by County Counsel and Risk Management, and return the vendor-executed Amendment to the Clerk of the Board for final execution by the Chair of the Board of Supervisors.
