Fiscal Impact/Change to Net County Cost: There is no Change to Net County Cost.

Background: Since the early 1990s the Mental Health Department has applied for and received a PATH grant each fiscal year to provide for the responsible and efficient transition of homeless and resource-less persons from the Psychiatric Health Facility (PHF) to appropriate placement in the community.

Reason for Recommendation: If awarded the grant, Mental Health will receive $28,840 in funding for FY 2008-09 to provide for the responsible and efficient transition of homeless persons and those without resources leaving the Psychiatric Health Facility (PHF) to appropriate placement in the community. The Department uses State Mental Health Services Act (MHSA) and Realignment funds to make the required 1:3 match of $9,614. The grant funds portions of the salaries and benefits of one discharge planner, two case managers and two rehab workers that work to transition these clients into housing when they leave the PHF.
