There is no Net County Cost associated with this Agreement.  Sufficient appropriations were included in the Fiscal Year 2012-13 budget.
Background
Agreement 454-S1010 with Willow Glen Care Center for the provision of residential treatment services for mentally ill adults was approved and executed by the Purchasing Agent on May 25, 2010 with a maximum obligation of $57,470 and a term of April 15, 2010 to April 14, 2013.  The Purchasing Agent signed Amendment I on April 6, 2011, which incorporated new daily rates.  Amendment II was approved by the Board of Supervisors on June 14, 2011, increasing the maximum obligation to $275,000.  Amendment III was approved by the Board of Supervisors on September 4, 2012, increasing the maximum obligation to $359,000 with no change to term or scope of services. Amendment IV to Agreement 454-S1010 increases the maximum obligation to $540,000 due to higher than anticipated usage, makes incidental changes to update contact information, and extends the end term date from April 14, 2013 to June 30, 2013 to allow sufficient time for negotiation of a new Agreement.  
 
Reason for Recommendation
Approval of Amendment IV will ensure continuity of residential treatment services for mentally ill adults.
Action(s) to be taken following Board approval
1.      Chair to sign two (2) original Amendments IV to Agreement 454-S1010.
2.      Board Clerk's Office to return one (1) signed original Amendment IV to HHSA at 3057 Briw Road, Contracts Unit.
3.      HHSA to distribute documents as appropriate.
