Fiscal Impact/Change to Net County Cost:    There is no anticipated exchange of monies in this Memorandum of Understanding (MOU), however the MOU does include a provision for the County to bill Marshall Medical Center for costs incurred for conferring and administering the trauma designation and for developing and maintaining the County's trauma plan in an amount not to exceed $4,000.00 for the term of the MOU.
 
Background:             
On July 17, 2007, the El Dorado County Board of Supervisors adopted Resolution No. 189-2007 establishing a "Trauma Designation Fee" for all costs associated with American College of Surgeons (ACS) review in preparation for trauma center designation, including but not limited to costs for a site review team and expenditures by the EMS Agency to complete the review and designation process.  In addition, the Board approved Agreement  697-PHD0208 providing the vehicle for reimbursement to Marshall for a portion of those costs incurred under Resolution No. 189-2007.  That agreement has subsequently been completed and satisfied.   
 
In the course of participating with Marshall in the ACS review and assessment, the County EMS Agency has determined that it would be more efficient for the applicant to directly procure and pay for any applicable review and assessment in preparation for Trauma Center application.  Resolution No. xx-2009 allows for this change along with establishing that County may recover its costs incurred in the initial and continuing designations of trauma centers.
 
At this time, Marshall Medical Center has completed the process required by County for ACS review and assessment and is seeking Level III Trauma Center designation.   The new MOU 801-PHD1008 would designate Marshall Medical Center as a Level III Trauma Center facility for the term of two years.
 
Reason for Recommendation:      
The ACS categorizes levels of trauma services in four tiers, with Level IV the lowest and Level I the highest.   Approval of the Agreement 801-PHD1008 would allow Marshall Medical Center to treat some trauma patients who otherwise must be transported to a trauma center in Sacramento or Roseville.  Adoption of Resolution xx-2009 would establish the authority for the County to assess Trauma Designation Fees in the future pursuant to California Health and Safety Code and cites that Resolution No xx-2009 supersedes Resolution No 189-2007.
 
Action to be taken following Board approval:
1. Chairman to sign Resolution.
2. Board Clerk's Office to return one (1) certified Resolution.
3. Chairman to sign three (3) original MOU's (Agreement #801-PHD1008).
4. Board Clerk's Office to return two (2) original MOU's to Department.
5. Department to distribute documents as appropriate.
