Background/Reason for Recommendation:  On July 22, 2008, Agenda item #14 your Board approved Agreement 1034-S0811 with Five Star Automotive to provide "as needed" automotive services for the Department of General Services.  The term of the Agreement was originally May 1, 2008 through April 30, 2010.  However, after execution of the contract, invoices were received from the vendor for services that had been rendered during the month of April.  In addition, the contract did not include a fee schedule for work that was sublet by the vendor.

This retroactive Amendment I to the Agreement 1034-S0811 corrects the term of the contract to reflect the  beginning date of services as  April 1, 2008 and includes outsourcing by vendor.

Action to be taken following Board approval: The Chairman to sign Amendment I to Agreement 1034-S0811. Board Clerk will forward the Amendment to General Services contract staff for further processing.
