Fiscal Impact/Change to Net County Cost: There is no Net County Cost associated with this Agreement 09-11270; in addition, this A01 does not change the original amount of the funding Agreement.

Background: The Health Services Department has historically received funding from the State on an annual basis to be used for the County’s Immunization Program. The original Agreement 09-11270 was approved by the Board June 30, 2009 via Board item 09-0828; it was mailed to the State, and the fully executed Agreement was received from the State November 24, 2009.

Retroactive Term: This Amendment 1 (A01) is retroactive to July 1, 2009. On June 9, 2010, Department received an inquiry from the State regarding the status of A01. According to the State, it was mailed to the Department on November 24, 2009; however, extensive review of e-mails, contract databases and files, and other documentation indicates that no such Amendment was ever received by the Department. In light of the late receipt of this Amendment A01 to that Agreement, Health Services Department has taken steps to ensure compliance with the Terms and Conditions of Amendment A01 to Agreement 09-11270.
