Health Services Department, Mental Health Division, recommending the Board authorize the Chair to sign Amendment I to Agreement 676-S0911 with Sacramento Children's Home, extending the term by one year through June 30, 2011 and increasing the compensation amount by $200,000 to $300,000 to provide specialty mental health services for seriously emotionally disturbed minors on an as requested basis for said Department.
FUNDING: Funding is provided by Medi-Cal/Early and Periodic Screening, Diagnosis and Treatment and Realignment, as determined by the funding sources available and appropriate for each youth placed in this facility. The agreement can be terminated within 7 calendar days without cause upon written notice should the funding be reduced or eliminated at the State/Federal or local level.

Fiscal Impact/Change to Net County Cost: The amount of this Amendment is $200,000 for a total contract amount of $300,000. The Mental Health Division is responsible for paying the contract provider the total amount due for specialty mental health services and subsequently billing the State for Medi-Cal//Early and Periodic Screening, Diagnosis and Treatment and Realignment (EPSDT) reimbursement. Year to date, the clients at Sacramento Children's Home have been 95% Medi-Cal eligible and the reimbursement rate for Medi-Cal/EPSDT is approximately 93%. Any amounts not reimbursed by Med-Cal/EPSDT are currently funded by Realignment. There is no Net County Cost associated with this Agreement.
Compensation for Services: Payments are made to contractor based upon monthly invoicing at specified billing rates for actual services performed and documented per contract requirements. Services and rates are detailed on Exhibit C, the last page of the original contract.
Reason for Recommendation: Sacramento Children's Home is a Level 12 Group Home, and provides support and supervision 24 hours per day. Clients are placed through Department of Human Services and the funding for residential services is provided under a separate agreement between the vendor and the Department of Human Services. This contract provides reimbursement for specialty mental health services to youth placed in this facility. Specialty mental health services provided under this Agreement include Case Management, Medication Support, Therapeutic Behavioral Services, Crisis Intervention, as well as individual and family therapy. An increase in the not-to-exceed amount is needed due to extension of the agreement for an additional year and our expectation of continued use of this group home.
The contract Amendment will allow the Department to continue providing specialty mental health services needed for seriously emotionally disturbed County youth. The Amendment will extend the Agreement for one additional year for a total term of July 1, 2009 through June 30, 2011.
The Purchasing Agent has determined that it is appropriate to dispense with any competitive selection process for services performed by Sacramento Children's Home as the Department contracts with multiple local area firms to provide similar services to its clients on an as needed basis. The Purchasing Agent concurs that the Departments request is in compliance with Board of Supervisors Policy C-17, Sections 7.5 and 7.10.
Action to be taken following Board approval:
1. Chairman to sign three (3) original Amendments.
2. Board Clerk's Office to return two (2) original Amendments to the Health Services Department, Mental Health Division.
3. Mental Health Division to distribute as appropriate.
