Fiscal Impact/Change to Net County Cost:   There is no Net County Cost associated with this funding Agreement.  The County will receive $49,157 through Sacramento County to support HOPWA activities in 2011.  In anticipation of this Agreement, the Health Services Department incorporated sufficient appropriations and revenue into the FY 2010-11 budget.

 

Background:   Sacramento County is the applicant/recipient for the Federal Housing and Urban Development (HUD) / HOPWA grant, and serves as the fiscal intermediary for the allocation of HOPWA funds.  El Dorado and other local rural counties receive HOPWA funding through Sacramento County. 

 

The HOPWA Program provides emergency housing assistance for County of El Dorado residents and their families living with HIV disease.  Short term assistance and supportive services are provided to qualified persons for rent, mortgage, and utilities to prevent homelessness.

 

HOPWA services have historically been contracted out to the Sierra Foothills AIDS Foundation, a community-based agency specializing in services for individuals and families afflicted with HIV/AIDS.  Sierra Foothills AIDS Foundation maintains all appropriate credentialing and has successfully passed annual site reviews.  The Department plans to contract with Sierra Foothills to provide services funded by this Agreement with Sacramento County for the term January 1, 2011 through December 31, 2011.

 

Compensation for Services:   Reimbursement for actual costs will occur as detailed on the Electronic Claim Form (ECF) claiming process provided by Sacramento County.  Claims shall be submitted by the 15th of each month.

 

Contract Termination:  This Agreement may be cancelled under the following conditions:  1) by either party without cause upon thirty (30) calendar days advance written notice to the other party; or 2) by Sacramento County immediately for cause should County of El Dorado fail to meet the terms, conditions and/or responsibilities of the Agreement.  In the event of termination prior to the end of the term of this Agreement, the County of El Dorado shall be paid for any services completed and provided prior to notice of termination, pursuant to Article XXXI, D.

 

Retroactive Term:   This Agreement was received from Sacramento County in November; the Department promptly began processing the Amendment through Department and County review processes.  All reasonable efforts were made to bring the Amendment before the Board as quickly as possible.

 

Reason for Recommendation:   Approval of this funding Agreement will allow the Department to continue providing HOPWA services to the community.

 

Action to be taken following Board approval:

1) Chair to sign three (3) original Agreements 101-162-P-R2011 along with three (3) original each of - Contractor Identification Form; Certification Regarding Debarment and Suspension Form; Contractor Certification of Compliance Form; and Certification Regarding Lobbying Form.

2) Chair to sign two (2) original copies of Resolution.

3) Board Clerk’s Office to return one (1) signed Resolution and three (3) signed Agreements to Department.

4) Department to forward all signed documents to Sacramento County.

5) Upon receipt of fully executed Agreements from Sacramento County, Department to return one (1) fully executed Agreement to Board Clerk’s Office.
