Health and Human Services Agency (HHSA) recommending the Board:
1) Approve and authorize the Chair to sign revenue-generating Agreement 9619 with California Health Collaborative, Inc. to accept clients to attend the El Dorado County Older Adult Day Services with a retroactive term of July 1, 2024, through June 30, 2026, with no stated maximum revenue;
2) Authorize the Purchasing Agent to execute amendments relating to revenue-generating Agreement 9619, contingent upon approval by County Counsel and Risk Management, which do not increase Net County Cost; and
3) Authorize the HHSA Director, or the Chief Assistant Director, to execute programmatic, administrative, and fiscal documents relating to Agreement 9619.
FUNDING: Fee-for-service revenue received from California Health Collaborative, Inc, funded by State of California General Fund and federal Medicaid programs.

DISCUSSION / BACKGROUND:
The California Department of Health Care Services (DHCS) Multipurpose Senior Services Program (MSSP) was established under the Welfare and Institutions Code (WIC) 9400 et seq. to address the fragmented delivery of health and social services for the frail elderly. The MSSP is a Medi-Cal program which provides services and supports for seniors who need extra help to stay in their homes as long as possible. The program aims to provide a coordinated network of care for individuals at risk of institutionalization by utilizing a range of vendors to deliver tailored services. Individuals who qualify for MSSP must meet the criteria for Frail Elderly status, which includes requiring assistance with three or more Activities of Daily Living (ADLs) and being eligible for Medi-Cal with no share of cost. CHC is the organization contracted with the DHCS to administer the MSSP.
HHSA’s Community Services Division operates an Older Adult Day Service Program known as "The Club", which serves older adults who need care and supervision during the day. The Club provides a warm, supportive, and engaging environment, offering a variety of activities designed to enhance physical and mental well-being, foster social connections, and improve overall quality of life. This setting helps participants remain active and involved, reducing isolation and promoting a sense of purpose. For families, The Club provides peace of mind and temporary relief from caregiving responsibilities, enabling seniors to remain at home and avoid institutional placement.
Through the proposed Independent Vendor Agreement (Agreement 9619) with CHC, the County may be eligible to receive reimbursement for the cost of services provided at The Club to low-income MSSP participants who are age 60 years or older. As a result of proposed Revenue Agreement 9619, CHC will refer MSSP clients to The Club and will reimburse HHSA for services provided to low-income participants. HHSA estimates that $25,000 will be received as County revenue through this agreement. CHC will also screen, enroll, and provide care planning for eligible El Dorado County MSSP clients. As HHSA was notified of the retroactive Agreement by CHC in April 2025, bringing this retroactive funding in agreement before the Board was unavoidable.
ALTERNATIVES:
Should the Board decline to approve this recommendation, the State’s MSSP contractor, California Health Collaborative, would be unable to refer local residents to the County’s Older Adult Day Services program.

FINANCIAL IMPACT:
Revenue from this Agreement will offset the cost of providing this service to participants. The rate included in the Agreement matches the rate approved by the Board in Spring 2025. The Club is funded by user fees and General Fund support.
