Fiscal Impact/Change to Net County Cost: The total not to exceed amount of this agreement is $95,000 for the two year contract period. There is no Change to Net County Cost. Services provided under this contract are Specialty Mental Health Services reimbursable through Medi-Cal generally, as well as pursuant to Medi-Cal's Early and Periodic Screening and Diagnosis (EPSDT) benefit. Approximately 90% of funding is provided by Medi-Cal, the public health insurance program for low-income individuals funded equally by the State of California and the federal government. Approximately 10% of funding comes from Realignment funds.
Background: Rupp Medical Corporation is the vendor through which the Mental Health Department obtains the services of Sonia Rupp, M.D. Dr. Rupp is a Board Certified Child and Adolescent Psychiatrist licensed to practice medicine in the State of California. She has provided psychiatric services for Mental Health Department to minors in the SLT area since 1999, including prescription of medication and monitoring of referred cases.
The services to be provided by Dr. Rupp to MHD are mandated entitlements and the state is legally required to pay for them in accordance with established formulas. Funding for the current fiscal year is secure.
Reason for Recommendation: This agreement will allow for continued use of Dr. Rupp's services to provide Specialty Mental Health Services for emotionally disturbed minors in El Dorado County. This agreement is retroactive to November 23, 2007. Delays in bringing the contract forward for approval were related to obtaining vendor insurance certification and signature.
The Board is being asked to find that it is more economical and feasible to obtain an outside contractor for the provision of psychiatric services to children and adolescents in the South Lake Tahoe (SLT) area than to provide them directly by County government. The ongoing aggregate of work is not sufficient to warrant the addition of permanent staff.
Action to be taken following Board approval: Board Clerk's Office will forward the executed agreement to Mental Health for further processing.
