Health and Human Services Agency (HHSA) recommending the Board:
1) Make findings in accordance with County Ordinance 3.13.030(C) that it is appropriate to contract with El Dorado County Community Health Center for the provision of Public Health services, provided under Funding Agreement 6498, because pursuant to County Ordinance 3.13.030(C) “the limited timeframes, temporary or occasional nature, or schedule for the project or scope of work, the ongoing aggregate of work to be performed is not sufficient to warrant addition of permanent staff;”
2) Approve and authorize the Chair to sign Funding Agreement 6498 with El Dorado County Community Health Center for the purposes of preventing and controlling vaccine-preventable diseases within the County, with a not-to-exceed (NTE) amount of $450,000, and for the term of upon execution through June 30, 2022 with the option to extend for up to two (2) one-year terms with no increase to the NTE; and
3) Authorize the HHSA Director, or designee, to execute further documents relating to Funding Agreement 6498, including amendments that do not increase the maximum dollar amount or term of the Agreement beyond June 30, 2024, contingent upon approval by County Counsel and Risk Management.
FUNDING: 100% California Department of Public Health Immunization Funding.

DISCUSSION / BACKGROUND:
Health and Human Services Agency (HHSA,) as the primary recipient of the California Department of Public Health (CDPH) Immunization Grant Agreement 17-10315 (Grant), has identified service needs that fall within the scope and purpose of the grant, and has submitted a budget and work plan to CDPH to sub award funds to a community partner for the purposes of preventing and controlling vaccine-preventable diseases within the County. El Dorado County Community Health Center (EDCCHC) is a community partner identified in the CDPH budget and work plan, currently providing community services within El Dorado County.
On January 21, 2022, Amendment 3 to the CDPH Grant Agreement 17-10315 was fully executed, increasing the payable amount to the County from $435,138 to $2,528,074 as authorized by Board Resolution 044-2021 on May 11, 2021. While the CDPH Immunization Grant Agreement 17-10315 has a term ending June 30, 2022, HHSA has received communication from CDPH indicating the funds will be carried over to FY 2022-23 and FY 2023-24.
To meet the scope and purpose of the Grant, the proposed subrecipient Funding Agreement #6498 with EDCCHC agrees to furnish personnel, equipment, supplies, travel and other materials, as needed to (1) Provide access to immunizations for vaccines recommended by the Advisory Committee on Immunization Practices (ACIP) to protect the local population; (2) Assist in detecting, reporting, and controlling vaccine-preventable diseases in its service area; and (3) Continue Vaccine For Children (VFC) Provider status, including vaccine management, storage and handling requirements, and data submission to the state vaccine registry, the California Immunization Registry (CAIR).
The initial term of the subrecipient Funding Agreement #6498 is limited by the term of the current CDPH Immunization funding agreement. Should the CDPH Immunization funding be carried into FY 2022-23 and FY 2023-24, the option to extend the term of this subrecipient agreement will be exercised, not to extend beyond June 30, 2024.
In accordance with Board of Supervisors Procurement Policy C-17, Section 7.11, the contractor selection process for service contracts in excess of $100,000 must include a review of the scope of services and contractor’s professional qualifications by a group of individuals (including at least one representative from outside the department requesting the services) qualified to judge the contractor’s ability to perform the services. HHSA has documentation on file that such a review was performed for Funding Agreement #6498 with EDCCHC.
ALTERNATIVES:
Should the Board decline to approve this recommendation, HHSA would continue to provide vaccine-preventable disease mitigation activities to both children and adults through the established Vaccine for Children Program and those mandates historically associated with Title 17 communicable disease response, mitigation, and control. While services would continue, capacity of response, volume of intervention, and efficiency mitigation/control would all be diminished, thereby increasing and prolonging disease exposure to the public. There is also the possibility of defaulting on the agreement with the State resulting in a loss of revenue and potentially jeopardizing future funding awards.

FINANCIAL IMPACT:
There is no Net County Cost associated with this Agenda item. Sufficient appropriations were included in the Fiscal Year 2021-22 budget, and will be included in future budgets for the term of the Agreement, if extended.
