Fiscal Impact/Change to Net County Cost: The amount of this Amendment is $31,500 for a total contract amount of $90,000. There is no Net County Cost associated with this Amendment II.
Compensation for Services: Payments are made to Contractor based upon monthly invoicing at specified billing rates for actual services performed and documented per contract requirements. Services and rates are detailed on page 5 of the contract.
Contract Termination: This Agreement includes the County's standard Fiscal Considerations termination language, as well as a provision for the County to terminate the Agreement without cause upon seven (7) calendar days written notice.
Background: One of the programs identified in the County's Board approved MHSA funded Prevention and Early Intervention (PEI) Plan is a Wellness Outreach Program, addressing vulnerable adults who have risk factors associated with suicide and/or suffer from mental distress, depression and/or exposure to trauma and isolation. The PEI Plan recognizes the need to provide outreach, engagement, screening, assessment, early intervention mental health services, and linkage to appropriate community based organizations for vulnerable adults who may have limited support systems.
The goals of the Turning Point program are to:
1. Provide early detection and increased access to screening, assessment, early mental health intervention and linkage to needed services and supports.
2. Provide linkage to the appropriate level of mental health and other needed services.
3. Provide training knowledge and skills related to mental health treatment, engagement and services, thereby promoting mental health and independent living.
4. Provide services in a proactive and community-based model, thereby reducing disparities in service access, utilization, and outcomes for vulnerable and isolated adults.
Reason for Recommendation: The population currently being served under this contract present with a higher risk and greater complexity of needs than previously anticipated. The contract Amendment will allow the Department to continue providing services for the Wellness Outreach Program for Vulnerable Adults on the Western Slope.
This Agreement covers the period March 11, 2011 through December 31, 2011.
Action to be taken following Board approval:
1. Chair to sign three (3) original Amendments.
2. Board Clerk's Office to return two (2) original Amendments to the Health Services Department, Mental Health Division.
3. Mental Health Division to distribute as appropriate.
