DEPARTMENT RECOMMENDATION:
Health and Human Services Agency (HHSA) recommends approval of Facility Use Agreement (FUA) 013-O1611 with the Mother Lode Lions Club (Lions Club) as it has been identified as the optimal site for the Senior Nutrition Program to continue to provide meals, five days a week, to those eligible seniors in a group setting.  This congregate meal program not only provides meals, but also encourages socialization, recreation, and activities to eligible senior citizens in the Diamond Springs location.  
 
Existing FUA 744-L0711 with the Lions Club expires June 30, 2015.  The Missouri Flat Road facility in Diamond Springs is one of six locations on the West Slope that is suitable to meet the needs of the eligible senior residents in the area and has been identified as the most suitable location in the Diamond Springs area.
 
DISCUSSION / BACKGROUND:
Established under the Older Americans Act, the Federally-funded Nutrition Program provides a vital link in maintaining the health of older persons by preventing premature institutionalization and improving overall quality of life.
 
HHSA has used the Lions Club, Diamond Springs location for the Senior Nutrition Program since the 1980s.  Approximately 350 eligible seniors per year are served at this location.  Recently, the facility underwent extensive renovations to update and repair the building.  While it was closed for those renovations, the Placerville Fairgrounds was used as a temporary location via contract with the El Dorado County Fair Association.  However, the Fairgrounds are not available year round and the Lions Club location is available to seniors located in that part of the County on a more consistent basis.
 
The Senior Nutrition program provides the daily contact and safety-net that older adults need when faced with declining mental and physical health, ensuring a social connection to ward off loneliness and depression that can follow the loss of independence through a decline in physical health or the loss of a spouse.
 
The monthly compensation amount is $1,800 for the period of July 1, 2015 through June 30, 2016, and includes an annual increase of three percent (3%) effective July 1, 2016 increasing the monthly compensation to $1,854 for the period of July 1, 2016 through June 30, 2017; and to $1,909 per month for the period of July 1, 2017 through June 30, 2018.  FUA  013-O1611 includes two additional three-year options to extend the term, with the same terms and conditions as the initial term, including the annual 3% increase.  
 
ALTERNATIVES:
Evaluation of possible sites in the area revealed there are very few suitable sites for a Senior Nutrition program in the County.  Potential sites such as the Placerville Fairgrounds are not available consistently on a year-round basis and are therefore not an adequate alternative to the Mother Lode Lions Club, Diamond Springs location.
 
OTHER DEPARTMENT / AGENCY INVOLVEMENT:
Concurrence by County Counsel and Risk Management.  Facilities Management negotiated the terms.  Procurement and Contracts developed the FUA.

FINANCIAL IMPACT:
The cost to the County is included in HHSA's Fiscal Year (FY) 2015-2016 Recommended Budget.  The County's General Fund contribution to Aging and Adult Services Programs is determined during the budget process.  
 
CLERK OF THE BOARD FOLLOW UP ACTIONS
1.  Clerk of the Board to obtain the signature of the Chair on two (2) originals of Facility Use Agreement 013-O1611.
2.  Clerk of the Board to return one (1) fully executed Facility Use Agreement to HHSA at 3057 Briw Road.
