Title
Health and Human Services Agency, Mental Health Division, recommending the Board:
1) Authorize the Chair to sign Amendment 4 to Agreement for Services 011-S1111 with Crestwood Behavioral Health, Inc., to provide long-term 24-hour special treatment programs for adults with serious mental disorders, and access to psychiatric health facility services on an "as requested" basis, extending the end term from June 30, 2013 to December 31, 2016 and increasing the not-to-exceed amount by $2,306,000 to $3,781,000, with a maximum obligation of $735,000 per year during Fiscal Years 2013/2014, 2014/2015 and 2015/2016; and
2) Authorize the Purchasing Agent to execute further documents relating to the above noted Agreement, including amendments thereto, contingent upon approval by County Counsel and Risk Management, which do not affect the maximum dollar amount or term of the Agreement.

There is no Net County Cost associated with this Agreement. Sufficient appropriations are included in the Fiscal Year 2013-14 Budget Request and will be included in future Fiscal Year Budget Requests for the term of this Agreement.
Background
Crestwood Behavioral Health, Inc. ("Crestwood") offers multiple levels of mental health services including 1) Mental Health Rehabilitation Center ("MHRC") facilities licensed by the State of California to provide twenty-four (24) hour intensive support and rehabilitation services designed to assist clients 18 years or older with mental disorders in the development of skills to become self-sufficient and capable of increasing levels of independent functioning; and 2) psychiatric health facility ("PHF") services. The Health and Human Services Agency Mental Health Division ("HHSA") does not have long-term residential facilities and must contract for these services. In addition, this Agreement for Services 011-S1111 provides access to the Crestwood PHF facilities in the event there is insufficient capacity at the County PHF.
On July 27, 2010, the Board of Supervisors approved Agreement for Services 011-S1111 (Agenda Item 10-0752) with a term of July 1, 2010 through June 30, 2013 and a maximum obligation of $1,025,000. On November 15, 2011, the Board of Supervisors approved Amendment I (Agenda Item 11-1263) updating the Program Description and Rate Schedule and with no change to the term or the maximum obligation of the Agreement. On July 17, 2012, the Board of Supervisors approved Amendment II (Agenda Item 11-1263) updating the Scope of Services and Rate Schedule, with no change to the term or the maximum obligation of the Agreement. On December 11, 2012, the Board of Supervisors approved Amendment III (Agenda Item 11-1263) increasing the maximum obligation by $450,000 to $1,475,000 with no change to the term.
Reason for Recommendation
Approval of this Amendment IV to Agreement for Services 011-S1111 will help to ensure availability of MHRC and PHF-related services for clients of the County on an "as requested" basis.
Action(s) to be taken following Board approval
1. Chair to sign two (2) original Amendments IV to Agreement for Services 011-S1111.
2. Clerk of the Board to return one (1) fully executed original Amendment IV to HHSA at 3057 Briw Road.
3. HHSA to distribute documents as appropriate.
