Title
Health and Human Services Agency, Public Health Division, recommending the Board consider the following:
1) Approve and authorize the Chair to sign documents required by the California Department of Public Health, Tuberculosis Control Branch, that will allow the County to receive reimbursement in the amount of $5,287 for core tuberculosis control and prevention activities to be performed during the period July 1, 2014 through June 30, 2015, and contingent upon approval by County Counsel and Risk Management;
2) Approve and authorize the Chair to sign any revised Real-Time Allotments, if issued, and contingent upon approval by County Counsel and Risk Management; and
3) Authorize the Director of the Health and Human Services Agency to administer the Real-Time Allotment and execute invoices submitted for reimbursement and required fiscal and programmatic reports.
FUNDING: California Department of Public Health, Tuberculosis Control Branch and Federal Funds.

Background
Tuberculosis (TB) is a disease caused by mycobacteria that primarily causes infection of the lungs, but can also cause disease in other parts of the body such as the brain, kidneys, or spine. Pulmonary TB is spread from person to person through airborne droplets of mycobacterium that are inhaled by a susceptible individual. All suspected and confirmed cases of TB must be reported by the medical community to their local Public Health agency to ensure proper investigation of the initially diagnosed individual (index case) and all associated contacts. The Public Health Division is also responsible for the coordination and facilitation of appropriate treatment and containment of further potential community infection.
Federal financial assistance is provided to local TB programs through the California Department of Public Health to ensure that core TB prevention and control activities at the local level are met to include case finding, initiation and completion of appropriate drug therapy; screening of persons who have had contact with TB-infected individuals, maintaining TB surveillance, community education, and TB specific public health laboratory services. These core activities are essential to effective TB prevention and control, and are consistent with the statutory authorities outlined below.
Health and Safety Code Sections 121350, 121355, 121357, 121158, 121360, 121375, 121380, 121390, 121450, 121455 and 121460 authorize the TB allocation program and charge the California Department of Public Health, Tuberculosis Control Branch, (CDPH TBCB) with establishing standards and procedures on which to condition the awarding of funds as mandated under California Health and Safety Code "Powers and Duties of Local Health Officers and Local Health Departments," Sections 101029-101040.
The Health and Human Services Agency (HHSA), Public Health Division (PH), has sporadically received an allotment from the CDPH TBCB since 2008. The purpose of the allotment is to reimburse HHSA for allowable intervention expenditures including but not limited to provision of TB medication, food, shelter, incentives, and enablers used to encourage individuals to complete treatment, prevent homelessness, and/or promote the least restrictive intervention in lieu of civil detention for the protection of the public at large.
Reason for Recommendation
Approval of the FY 2014 - 2015 TBCB Real-Time Allotment will allow HHSA, through its Public Health Communicable Disease Program, to be reimbursed for certain core TB control and prevention activities performed by HHSA. These funds are intended to provide HHSA with increased capabilities to address TB control needs. This increased capability is essential in providing the proper medical treatment to identified infected individuals, which, if started soon enough and completed, can potentially cure the person infected with TB. Curing those with an infectious treatable disease also protects the health and well-being of the community of El Dorado County.
