DEPARTMENT RECOMMENDATION

Health and Human Services Agency (HHSA) recommending the Board approve and authorize the Chair to sign Amendment 2 to Agreement for Services 086-S1511 with Progress House, Inc.  HHSA receives funding under an Agreement with the California Department of Health Care Services (DHCS) for the provision of Substance Use Disorder prevention and treatment, which mandates HHSA to make substance abuse treatment services available to Medi-Cal beneficiaries.   Approval of this Amendment is recommended to continue the provision of mandated alcohol and drug counseling and treatment services for both existing and future clients, as well as to add a new detoxification service.  Failure to contract with community-based alcohol and drug treatment providers would result in an inability to meet the service demands of the community, as well as an inability to provide the service levels mandated by the funding agreement.  HHSA recommends approval of Amendment 2 to allow continued provision of alcohol and drug counseling and treatment services for both existing and future clients.

 

Additionally, HHSA has determined that in accordance with County Ordinance Section 3.13.030, the County has neither the facilities nor the staff available to meet the service requirements. 

 

DISCUSSION / BACKGROUND

The Substance Abuse and Mental Health Services Administration (SAMHSA) is a branch of the U.S. Department of Health and Human Services and is charged with improving the quality and availability of prevention, treatment, and rehabilitative services to reduce illness, death, disability, and cost to society resulting from substance abuse and mental illness.  SAMHSA offers many grant opportunities, including the Substance Abuse Prevention and Treatment (SAPT) Federal block grant, which is mandated by the U.S. Congress to be used to fund substance abuse and mental health services.  In California, the public system of care for the prevention and treatment of Substance Use Disorder (SUD) is overseen by DHCS and is administered by counties.  DHCS annually receives SAPT, Medicaid, and State Behavioral Health Realignment funding that it makes available to counties to provide these mandated substance abuse prevention and treatment services.

 

The County of El Dorado Board of Supervisors approved revenue Agreement 284-F1511 (DHCS 14-90055) with DHCS on December 2, 2014 for the provision of SUD services during the term of July 1, 2014 through June 30, 2017 (File ID 14-0612, Item 13).  One of the priorities of the HHSA is to provide an array of evidence-based services designed to prevent and reduce alcoholism and drug addiction.  HHSA’s SUD programs accomplish this mission through education, prevention, treatment, and counseling services offered directly through contracts with community agencies.

 

On September 30, 2014, the Board of Supervisors approved Agreement for Services 086-S1511 with Progress House, Inc. for the term September 30, 2014 through June 30, 2016 and a maximum contractual obligation of $1,555,000 (File ID 14-0930, Item 29, continued from September 16, 2014, Item 15).  On June 16, 2015, the Board of Supervisors approved Amendment 1 with Progress House, Inc., which extended the term through June 30, 2017 and increased the maximum contractual obligation to $2,501,000.  The State recently licensed Progress House to operate a six-bed detoxification facility.  This service will be invaluable to clients as they begin to move toward sobriety.  Amendment 2 adds detoxification services and increases the maximum contractual obligation by $126,000 for a maximum obligation of $2,627,000.  

 

In addition to Progress House, Inc., HHSA contracts with other community-based organizations outside of Placerville for the provision of alcohol and drug program services.  Maximum contractual obligations are determined based upon historical financial analysis, utilization, service availability, location, and consideration of other factors such as changes in the Drug Medi-Cal services and client Medi-Cal eligibility.

 

ALTERNATIVES

Disapproval of Amendment 2 to Agreement for Services 086-S1511 will result in difficulty meeting the service demands of the community, may disrupt the progress and continuity of care for clients currently receiving services from Progress House, and based on current demand will result in an inability to provide the service levels mandated by the funding agreement.

FINANCIAL IMPACT

There is no Net County Cost associated with this Agreement.  Sufficient appropriations were included in the fiscal year 2015-16 budget, and will be included in future budgets for the term of the Agreement.

1. Clerk of the Board to obtain signature of Chair on two (2) original Amendment 2 to Agreement for Services 086-S1511.
