Health and Human Services Agency (HHSA) recommending the Board:
1) Make findings that the provision of public health immunization services provided by Subrecipient are in the public's best interest due to limited timeframes, temporary or occasional nature, or schedule for the project or scope of work, the ongoing aggregate of work to be performed is not sufficient to warrant addition of permanent staff, in accordance with El Dorado County Ordinance Code, Chapter 3.13.030(C);
2) Approve and authorize the Chair to sign Amendment I to Funding Agreement 6593 with Marshall Medical Center, which extends the term by one year, for a new term of September 27, 2022, through June 30, 2025, with no change to the maximum obligation;
3) Approve and authorize the Chair to sign Amendment II to Funding Agreement 6419 with Barton Healthcare System, which extends the term by one year, for a new term of April 12, 2022, through June 30, 2025, and reduces the maximum obligation from $350,000 to $100,000;
4) Authorize the Purchasing Agent to execute amendments relating to Funding Agreements 6593 and 6419, contingent upon approval by County Counsel and Risk Management, which do not increase the maximum dollar amount or term of the Agreement; and
5) Authorize the HHSA Director, or the Chief Assistant Director, to execute programmatic, administrative, and fiscal documents relating to Funding Agreements 6593 and 6419.
FUNDING: 100% California Department of Public Health Immunization Funding.

DISCUSSION / BACKGROUND:
Beginning in 2017, HHSA’s Public Health Division (PHD) has been the primary recipient of the CDPH Immunization Grant, awarded initially under Grant Agreement 17-10315 (Fenix 6228) with a subsequent award granted under Grant Agreement 22-11034 (Fenix 7040), with the current grant’s term through June 30, 2027. The purpose of this grant is to assist local health departments prevent and control vaccine preventable diseases (VPD) in their local health jurisdictions.
As required to receive said grants, HHSA PHD identified service needs falling within the scope and purpose of the Immunization Grants, and subsequently submitted a budget and work plan to CDPH to allocate these funds to community partners. HHSA’s PHD aim was to aid in preventing and controlling vaccine-preventable diseases within the County. Marshall Medical Center and Barton Healthcare System were community partners identified in the CDPH Immunization Grant budget and work plan and have actively been providing community services within El Dorado County.
On April 12, 2022, the Board approved Funding Out Agreement 6419 with Barton Healthcare System, for a term upon execution through June 30, 2024, with a maximum obligation of $350,000 (Legistar File 22-0506). Subsequently, on September 27, 2022, the Board approved Funding Out Agreement 6593 with Marshall Medical Center for the purpose of preventing/controlling VPDs in the County, for a term upon execution through June 30, 2024, with a maximum obligation of $350,000 (Legistar File 22-0851).
As stated in the scope of both Funding Out Agreements 6593 and 6419, both subrecipients will furnish personnel, equipment, supplies, travel and other materials, as needed to: (1) Provide access to immunizations for vaccines recommended by the Advisory Committee on Immunization Practices (ACIP) to protect the local population; (2) Assist in detecting, reporting, and controlling vaccine-preventable diseases in its service area; and (3) Continue Vaccine For Children (VFC) Provider status, including vaccine management, storage and handling requirements and data submission to the state vaccine registry, the California Immunization Registry (CAIR).
HHSA recommends that the Board approve this agenda item, enabling PHD to better align with the terms of Funding Out Agreements 6419 and 6593 under the CDPH Immunization Grant 22-11034, Rounds 2-4 (COVID portion). The funds from these agreements can be utilized until June 30, 2025. The reduction of the Barton Healthcare System Agreement 6419 maximum subaward obligation was a mutual decision between HHSA PHD and the subrecipient to align the funding award amount with the subrecipient’s planned grant expenditures.
ALTERNATIVES:
Should the Board decline to approve this recommendation, HHSA will have Barton Healthcare System and Marshall Medical Center continue to provide services agreed to under Funding Out Agreements 6593 and 6419 through the original term end dates of June 30, 2024.

FINANCIAL IMPACT:
There is no Net County Cost associated with this agenda item. Sufficient appropriations were included in the Fiscal Year 2024-25 Budget and will be included in future budgets for the terms of the agreements.
