Public Health Director requesting authorization to sign Amendment I to Cost Reimbursement Agreement 7275-07/08-709 A-1 with the County of Sacramento increasing the reimbursable amount by $106,935 to $160,403 for a term through February 29, 2008 to provide direct emergency services through The Ryan White C.A.R.E. Act. to people living with HIV disease.
RECOMMENDED ACTION: Approve.

Fiscal Impact/Change to Net County Cost: This Ryan White C.A.R.E. Act Agreement provides an additional $106,935 in funding for the term 3/01/07 through 2/29/08. The FY 06/07 adopted budget included funding in the amount of $53,468 as did the FY 07/08 County budget. There is no net County cost.
Background: The Ryan White C.A.R.E. Act provides direct emergency services for people living with HIV disease. Services include, but are not limited to, information, referrals and/or arranging for outpatient medical care, skilled nursing, medication management, diagnosis and treatment of common medical and mental conditions, and addressing nutrition issues to include direct food purchases as well as support to local food banks in El Dorado County. El Dorado County Ryan White and HOPWA (housing assistance) services are subcontracted out to Sierra Foothills AIDS Foundation, a community based agency specializing in services to individuals and families afflicted with AIDS. Sierra Foothills AIDS Foundation maintains all appropriate credentialing and has successfully passed annual site reviews.
Reason for Recommendation: El Dorado and other local rural counties obtain their State sponsored Ryan White C.A.R.E. Act and HOPWA funding through Sacramento County.
Action to be taken following Board approval: Public Health Director to sign five (5) originals of attached Amendment and forward to Sacramento County for final execution; Public Health to return one fully executed original Amendment to Board Clerk's Office.
