Probation Department recommending the Board:
1) Accept grant funding in accordance with Board Policy A-6, in an amount of $300,000, from the California Department of Health Care Services for the California Providing Access and Transforming Health (PATH) Justice-Involved Planning and Capacity Building Program Round 4;
2) Authorize the Chief Probation Officer to complete and submit the Acknowledgement of Grant Terms and Conditions to the Department of Health Care Services (DHCS); and
3) Delegate authority to the Chief Probation Officer, or Deputy Chief Probation Officer, to sign certifications on invoices to receive reimbursement for activities and to execute any required fiscal and programmatic reports for PATH 4 funding.
FUNDING: 100% grant funded through the PATH funding for the CalAIM Justice-Involved Initiative.

DISCUSSION / BACKGROUND:
California Advancing and Innovating Medi-Cal (CalAIM) is a multi-year initiative, by the Department of Health Care Services (DHCS) to improve the quality of life and health outcomes of individuals on Medi-Cal by implementing a broad delivery system, as well as program and payment reform across the Medi-Cal program. The CalAIM initiative utilizes the Medi-Cal program to help address the multifaceted challenges facing California's most vulnerable residents such as individuals experiencing homelessness, behavioral health care access, children with complex care needs, the growing number of justice-involved populations who have significant clinical needs, and the growing aging population.
PATH is a $1.44 billion program approved under CalAIM. It provides transitional funding to invest in providers, counties, community-based organizations, and other partners to support the State’s efforts to maintain, build, and scale the capacity necessary to support and implement key initiatives under CalAIM.
On December 13, 2022, with Legistar file 22-2061, the Board accepted grant funding for PATH Round 1. The Probation Department’s (Probation) allocation was $50,000. On November 14, 2023, with Legistar file 23-2036, the Board accepted grant funding for PATH Round 2. Probation’s allocation was $250,000. On December 13, 2023, with Legistar file 23-1917, the Board accepted grant funding for PATH Round 3. Probation’s allocation was $1,500,000.
On July 11, 2025, Probation submitted an application to DHCS for PATH Round 4 funding. On April 22, 2026, the DHCS announced the Round 4 grant awards, including awarding $300,000 to Probation. As a condition of receiving the grant funds, Probation must warrant and guarantee that it will comply with all applicable federal, state, and local laws and regulations, as well as sign a Terms and Conditions document provided by DHCS.
Probation will use Round 4 grant funding for contract consulting to support fiscal management and oversight of Medi-Cal billing; contributing to contracted electronic health record and health information exchange services; and charging back 20% of the time Probation’s Mental Health Program Coordinator, Mental Health Clinician, and Health Educator spend on claimable activities in their current capacity.
ALTERNATIVES:
Should the Board decline to approve the PATH Round 4 grant funding, the County would be ineligible to participate in the Program and, as a result, not be provided the funding identified for El Dorado County.
