Fiscal Impact/Change to Net County Cost
There is no Net County Cost associated with this rate change.  It is anticipated this rate change will result in increased revenue.
Background
The Health and Human Services Agency (HHSA), on behalf of the County, contracts with the State to serve as the Mental Health Plan (MHP) for the County.  HHSA, as the MHP, provides a comprehensive system of mental health services for the residents of the County, including mandated services, through both County staff, and through contracted service providers, as appropriate.  The County's Psychiatric Health Facility (PHF) is an essential component of HHSA's system of mental health services.  The PHF is a 16-bed acute care facility designed to provide 24-hour psychiatric stabilization and treatment services for those persons who meet criteria under Welfare and Institutions Code 5150, which is defined as imminent danger to self or others, or gravely disabled due to a mental disorder.  In addition to serving HHSA clients, HHSA also contracts with the counties of Alpine, Amador, Calaveras, Colusa, Mono, Nevada, Plumas, Shasta, Solano, and Trinity to provide PHF services to their clients based on available beds.
 
For many years, the State provided counties a Short-Doyle/Medi-Cal Schedule of Maximum Allowance (SMA) rate in accordance with Welfare and Institutions Code Section 5720 (a) and (b).  The County of El Dorado also established published rates for services paid for by sources other than Medi-Cal, including the PHF rate per bed/day.  As of July 1, 2012, Assembly Bill 1297 eliminated the State's Short-Doyle/Medi-Cal Maximum Reimbursement Rate, which had effectively capped the County's per bed/day rates.  
 
The County PHF was increased from ten (10) to sixteen (16) bed capacity in 2013, resulting in increased number of clients served.  This proposed rate of $835 per bed day for the PHF is based on the actual units of service provided at the PHF for fiscal year (FY) 2013-14 and the projected actual cost of providing those services. This rate will allow HHSA to recover our cost of providing services to other counties while remaining competitive with other PHF providers.  It is anticipated that this increase in the rate will result in increased revenues of approximately $100,000 per fiscal year.
 
Reason for Recommendation
Approval of the attached proposed Resolution increasing the published rates for the bed/day rates for the PHF, will allow HHSA to bill based on actual cost of services.
