Health and Human Services Agency (HHSA) recommending the Board:
1) Make findings that the provision of Dignity at Home Fall Prevention Grant Program services provided by Contractor are in the public's best interest and that there are specialty skills, qualifications, and equipment not expressly identified in County classifications involved in the performance of the work in accordance with El Dorado County Ordinance Code, Chapter 3.13.030(B), El Dorado County Charter, Section 210(b)(6), and/or Government Code Section 31000;
2) Approve and authorize the Chair to sign Agreement for Services 6594 with Rebuilding Together Sacramento, with a not-to-exceed amount of $175,000, and for a term effective upon execution through December 31, 2024, including one (1) additional one (1) year extension to coincide with California Department of Aging, Agreement IF-2223-29; and
3) Authorize the HHSA Director, or the Assistant Director of Human Services, to execute further documents relating to Agreement 6594, contingent upon approval by County Counsel and Risk Management, including amendments which do not increase the maximum dollar amount or term of the Agreement.
FUNDING: 100% State of California Department of Aging Grant.

DISCUSSION / BACKGROUND:
Assembly Bill 74 of the State of California (State) Budget Act of 2019, Chapter 23 provides the legislation for the Dignity at Home Fall Prevention Program, a program provided to eligible disabled individuals sixty (60) years of age or older through Area Agencies on Aging via grants from the State. The Fall Prevention Programs provides fall risk assessments for persons deemed “at risk for falling” by medical providers or through an assessment process. The program provides in-home supports to prevent fall risk such as the purchase and installation of grab bars, shower chairs, mobility ramps, handrails, etc.
