DISCUSSION / BACKGROUND
The CalAIM initiative is a statewide comprehensive program that focuses on connecting incarcerated individuals to community-based care upon release, including Medi-Cal enrollment and access to behavioral and social services. Key aspects of the CalAIM Justice-Involved Initiative include re-entry planning, Medi-Cal enrollment, pre-release services, continuity of care, and multi-departmental collaboration. CalAIM’s primary initiative is to help incarcerated individuals prepare to reintegrate into the community post-release from jail. These services are critical to ensuring that formerly incarcerated individuals receive the necessary care and support they need to successfully reintegrate into the community, reduce recidivism, and improve long-term health outcomes.
