Title
Health and Human Services Agency, Public Health Division, recommending the Board:
1) Approve and authorize the Chair to sign Agreement for Services 408-S1611 with Healthy Communities Institute, for the continued provision of and technical support for the “Well Dorado” (www.welldorado.org) web-based platform, with an initial term of June 18, 2016 through September 30, 2019 and automatically renews for successive one-year terms in perpetuity, and with an annual not-to-exceed amount of $30,500 for the first three years of the term. After the first three years, the rate may increase by no more than 5% annually; and
2) Authorize the Purchasing Agent, or designee, to execute further documents relating to Agreement for Services 408-S1611, including amendments which do not increase the maximum dollar amount or term of the Agreement, and contingent upon approval by County Counsel and Risk Management.

DEPARTMENT RECOMMENDATION
Health and Human Services Agency (HHSA) recommends the Board approve the renewal agreement with Healthy Communities Institute. Healthy Communities Institute provides technical support for HHSA’s website, “Well Dorado” (www.welldorado.org). This website allows users, both internal and external to HHSA, to access data on a variety of health and community-related data, share best practices, and access community calendars for health-related events. The website also is an integral part of the Public Health Accreditation process.
Additionally, HHSA has determined, in accordance with County Ordinance Section 3.13.030, it is more economical and feasible to engage an independent contractor for the provision of web support and technical services because the Contractor owns and provides the web platform.
