Title
Health and Human Services Agency, Community Services Division, recommending the Board:
1) Adopt Resolution 018-2012 authorizing the Chair to sign Agreement 11-10461 with the California Department of Public Health, in the amount of $3,208,000 for the provision of Supplemental Food Program for Women, Infants and Children (WIC) services to eligible residents of El Dorado and Alpine counties during the term October 1, 2011 through September 30, 2014. The Resolution further authorizes the Director of Health and Human Services, or successor, or the Chief Assistant Director, or successor, to execute subsequent documents relating to said Agreement, including one or more amendments thereto, contingent upon approval by County Counsel and Risk Management, that do not affect the dollar amount or the term, to sign subsequent required fiscal and programmatic reports and to perform any and all administrative responsibilities in relationship to said Agreement; and
2) Authorize the Chair to sign the Local Government Name Change form as required by the California Department of Public Health to reflect the Agency’s name change from the Department of Human Services to the Health and Human Services Agency.

No change. The maximum amount payable by the State to the County of El Dorado under this Agreement is $3,208,000 for the three-year term of October 1, 2011 through September 30, 2014. The California Department of Public Health has allocated $952,000 for FFY 2011-12; $1,069,000 for FFY 2012-13 and $1,187,000 for FFY 2013-14. A portion of the funding is budgeted for FY 2011-12. The remainder will be budgeted in future fiscal years.
