Title: Health and Human Services Agency, Community Services Division, recommending the Board adopt Resolution 020-2012 authorizing the Chair to sign Amendment 4 to Agreement 10B-5608 with the California Department of Community Services and Development, extending the term to June 30, 2012, with no change in the total dollar amount of $1,485,007, for the provision of Low-Income Home Energy Assistance Program services to eligible residents of El Dorado and Alpine counties and further authorizing the Agency Director, or successor, or the Chief Assistant Director, or successor, to continue to administer Agreement 10B-5608 as amended and execute subsequent documents relating to said Agreement, including amendments thereto, contingent upon approval by County Counsel and Risk Management, that do not affect the total dollar amount or the term, and any subsequent required fiscal and programmatic reports. Fiscal Impact/Change to Net County Cost:  No change.  Federal funds payable by the State to the County of El Dorado for LIHEAP services are included in the Department's FY 2011-12 Budget.
 
Background:  On February 2, 2010 the Board adopted Resolution No. 009-2010 authorizing the execution of Agreement 10B-5608 with the California Department of Community Services and Development (CSD) in the amount of $326,434 during the term January 1, 2010 through April 30, 2010 for provision of Low-Income Home Energy Assistance Program (LIHEAP) services to eligible residents of El Dorado and Alpine counties.  Resolution No. 009-2010 further authorized the Department Director, or successor, to sign one or more amendments, contingent upon approval by County Counsel and Risk Management, that collectively increased the dollar amount by up to $1,733,266 and that extended the term to June 30, 2011.  Amendment 1 to Agreement 10B-5608 increased the agreement by $1,063,879, for a total amount of $1,390,313 and extended the term to June 30, 2011.  Amendment 2 to Agreement 10B-5608 increased the amount by $94,694 for a total amount of $1,485,007.  Both Amendment 1 and Amendment 2 were signed by the Director as authorized by Resolution 009-2010.  On May 24, 2011 the Board adopted Resolution 077-2011 authorizing execution of Amendment 3 to Agreement 10B-5608 extending the term to December 31, 2011.
 
Reason for Recommendation:  Amendment 4 to Agreement 10B-5608 extends the Agreement term through June 30, 2012 to allow full expenditure of the 2010 funding for provision of Energy and Weatherization Assistance services to eligible residents of El Dorado and Alpine Counties.  
 
Action to be taken following Board approval:
Board Clerk to provide Health & Human Services, at Briw Road, with:
1.      Two (2) certified resolutions.
2.      Two (2) certified minute orders.
3.      Two (2) originals of Amendment 4 to Agreement 10B-5608 signed by the Chair.
