Fiscal Impact/Change to Net County Cost:  There is no Net County Cost associated with this Agreement.  Sufficient appropriations due to salary savings from unfilled psychiatrist positions are available and were budgeted in Fiscal Year 2012-13 and will be budgeted for FY 2013-14 and FY 2014-15.
Background:  Agreement 140-S1211 with Clinicians Telemed Medical Group, Inc. in the amount of $260,000 for the term of August 9, 2011 through June 30, 2012 for psychiatry services on an "as requested" basis for the Health Services Department, Mental Health Division, was approved and executed by the Board on August 9, 2011.  Outside review of the contractor's qualifications was conducted in accordance with Board of Supervisors Policy C-17 for Agreement 140-S1211. The Procurement and Contracts Division concurs that the competitive selection process that resulted in the award of that Agreement with Clinicians Telemed Medical Group, Inc., has not expired.   
 
Compensation:  Payments are made to Contractor based upon monthly invoicing, at specified billing rates as detailed in the Agreement, for actual services performed and documented per contract requirements.
Contract Termination:  This Agreement includes the County's standard Fiscal Considerations termination language, as well as a provision for the County to terminate the Agreement without cause upon seven (7) calendar days written notice.
Reason for Recommendation:  County of El Dorado Health and Human Services Agency, Mental Health Division (HHSA/MHD), remains unsuccessful in its ongoing recruitment to fill 2.0 full time psychiatrist allocation vacancies.  As a result, HHSA/MHD has sought alternative solutions to ensure availability of psychiatrist services for the population that requires specialty psychiatry/medication as a core element of their treatment.  
 
Clinicians Telemed Medical Group, Inc. has successfully proven they are able to perform remote psychiatric services for HHSA/MHD by the use of video conferencing equipment from its offices located in Bakersfield, CA, thereby resulting in a beneficial relationship that meets the needs of HHSA/MHD and its clients.  An outside review was obtained in accordance with Board of Supervisors Policy C-17, and the outside reviewer concurs that the vendor is qualified and able to perform the services.
 
Approval of Agreement 423-S1211 for the term of July 1, 2012 through June 30, 2015 in the amount of $720,000 with CTMG will allow for continuing psychiatric services for eligible HHSA/MHD clients.  
 
Action to be taken following Board approval:
1.      Chair to sign two (2) original Agreements 423-S1211.
2.      Board Clerk's Office to return one (1) fully executed original Agreement.
3.      HHSA/MHD to distribute documents as appropriate.
