There is no Net County Cost associated with this funding application.  County match using MHD Realignment funds of $12,008 has been budgeted for FY 2012-13.
 
Background
The Board of Supervisors approved the PATH grant application for FY 2011-12 on June 14, 2011 via Board Item 12.
 
Reason for Recommendation
The Health and Human Services Agency Mental Health Division (HHSA/MHD) has applied for and received a grant through the Projects for Assistance in Transition from Homelessness (PATH) grant since the early 1990's.  This grant funds outreach services for seriously mentally ill adults who are at risk of homelessness, or are homeless, in their transition from the Psychiatric Health Facility (PHF) and Crisis Residential Treatment (CRT) facility into appropriate placement in the community.
 
HHSA/MHD received the funding application from the California Department of Mental Health on April 10, 2012 with a due date of May 11, 2012.  Because the PATH grant renewal application forms were not available from the State in time to obtain Board of Supervisors signature for submittal by the deadline, the Agency submitted the application and required certifications with the Director's signature rather than fail to obtain funding for continuation of these services. As required by Board Policy A-6, HHSA/MHD notified the Grantor that the application cannot be considered binding until ratified by the Board of Supervisors.  HHSA is recommending retroactive ratification of the Director's signature on the application and certifications.
 
Action to be taken following Board approval:
1) Board Clerk to provide two (2) Certified Minute Orders to HHSA/MHD at 931 Spring Street.
2) HHSA/MHD to submit Certified Minute Orders to California Department of Mental Health documenting ratification of the application by the Board of Supervisors.
