Health and Human Services Agency, Social Services Division, recommending the Board authorize the Chair to sign Agreement for Services 135-S1311 with Jill Gustafson, LCSW, for the provision of therapeutic counseling and therapeutic visitation services on an "as requested" basis, for the term of September 24, 2012 through September 23, 2015 in the not-to-exceed amount of $200,000.

No change. The Agreement is funded by Federal and State dollars with required County match met primarily with realignment resources. Funds were budgeted in Fiscal Year (FY) 2012-13 and will be budgeted in FY 2013-14, FY 2014-15, and FY 2015-16.
Compensation
Payments to contractor are made within forty-five (45) days following the County's receipt and approval of itemized invoices identifying services rendered. Compensation is based upon specified billing rates for actual services performed and documented per contract requirements.
Termination
Agreement for Services 135-S1311 includes the County's standard Fiscal Considerations termination language as well as a provision for the County to terminate the Agreement without cause in seven (7) calendar days upon written notice.
Reason for Recommendation
The County of El Dorado Health and Human Services Agency, Social Services Division ("HHSA") maintains agreements with multiple vendors for the provision of therapeutic counseling and therapeutic visitation services to ensure clients can be referred by their caseworkers on an "as requested" basis to receive necessary services from the most appropriate and conveniently located vendor.
