Title: Health and Human Services Agency, Community Services Division, recommending the Board adopt Resolution 119-2014 to: 1) Authorize the Chair to sign Agreement MS-1415-35 in the amount of $257,100 and the accompanying Contractor/Vendor Confidentiality Statement with the California Department of Aging for the provision of the Multipurpose Senior Services Program during the term July 1, 2014 through June 30, 2015; 2) Authorize the Chief Administrative Officer, or designee, to execute any future amendments thereto, contingent upon approval by County Counsel and Risk Management, that do not affect the total dollar amount or the term; and 3) Further authorize the Director of the Health and Human Services Agency, or designee, to continue to administer Agreement MS-1415-35 and execute any subsequent documents relating to said Agreement including required fiscal and programmatic reports. There is no Net County Cost associated with this Agreement.  The amount payable by the State to the County of El Dorado under this Agreement is $257,100 with no required match.  Sufficient appropriations were included in the Fiscal Year (FY) 2014/15 Recommended Budget.  
 
Background
Under a federal Medicaid Home and Community-Based, Long-Term Care Services Waiver, the Multipurpose Senior Services Program (MSSP) provides social and health care management to allow frail elderly clients to remain at home. Clients eligible for the program must be 65 years of age or older, live within El Dorado County, be appropriate for care management services, be currently eligible for Medi-Cal, and certified or certifiable for placement in a nursing facility. On-going and frequent contact between MSSP staff and program participants ensures required care is received for the client to remain safely in their own home. The County of El Dorado, through its Department of Community Services (now part of the Health and Human Services Agency) has administered the program locally since funding was first awarded in 1999.  
 
Reason for Recommendation
Agreement MS-1415-35 provides $257,100 in federal and state funding for continued provision of MSSP services in the El Dorado County during FY 2014-15.
