Title: Health and Human Services Agency, Community Services Division, recommending the Board consider the following for the Senior Day Care Program: 1) Accept a grant award in the amount of $400 from the Friends of El Dorado County Seniors; 2) Approve and authorize the Chair to sign a retroactive Agreement for Funding 094-F1511 with the Friends of El Dorado County Seniors, a fund at the El Dorado Community Foundation, for said grant funds in the amount of $400 during the term July 1, 2014 through June 30, 2015; and 3) Approve and authorize the Chair to sign a retroactive Agreement for Funding 093-F1511 with the Women’s Fund El Dorado for grant funding in the amount of $1,447.79 during the term July 1, 2014 through June 30, 2015. Background
The Health and Human Services Agency (HHSA) Senior Day Care (SDC) Program is designed to meet the needs of adults who are homebound or isolated due to a stroke, Alzheimer's disease, or other debilitating mental or physical condition.  The Senior Day Care Program provides its participants with enriching educational, therapeutic, and social experiences outside the home and offers respite for caregivers.  The County has operated the Senior Day Care Program in Placerville since 1989 and opened a satellite Senior Day Care site in El Dorado Hills on March 31, 2014.  
 
SDC offers participation in the Elder ID program and Care TrakTM program to families of individuals with dementia who wander.  Services include registration with local law enforcement agencies and the SDC program, maintenance of identification information, and the lease of tracking equipment.  On April 29, 2014, HHSA submitted a grant application to the Friends of El Dorado County Seniors requesting funds in the amount of $660.58 to fund printing and mailing of program brochures and the purchase of seven C12 transmitter testers for the Care TrakTM program tracking equipment.  The Friends of El Dorado County Seniors awarded funding in the amount of $400 for the purchase of the testers which will ensure the tracking equipment available for lease is functioning properly.
 
On June 17, 2014, the Board of Supervisors accepted a grant award in the amount of $1,447.79 from the Women's Fund El Dorado for the purchase of a washer/dryer combination at the El Dorado Hills Senior Day Care satellite center.  
 
Reason for Recommendation
Acceptance of the award from the Friends of El Dorado County Seniors and approval of Agreements 093-F1511 and 094-F1511 will allow the SDC program to use the grant funds for the purchase of the designated equipment.

Provide Health and Human Services at Briw Rd. with the following signed by the Chair:
1) One (1) original of Agreement 094-F1511; and
2) One (1) original of Agreement 093-F1511.
