There is no Net County Cost associated with this Agreement.  Sufficient appropriations were included in the Health and Human Services Agency (HHSA) FY 2014-15 Budget and will be included in FY 2015-16 Budget.
 
Background
On May 13, 2014, the Board of Supervisors accepted an allocation of $100,000 from the California Department of Health Care Services (DHCS) for Medi-Cal Outreach and Enrollment activities (Agenda item 33, File ID 14-0597).  The purpose of the grant is to provide Medi-Cal Outreach and Enrollment (O&E) services and activities pursuant to Assembly Bill 82, Section 71, which focus on serving targeted populations such as:
1)  Persons with mental health disorder needs.
2)  Persons with substance use disorder needs.
3)  Persons who are homeless.
4)  Young men of color.
5)  Persons who are in county jail, state prison, on state parole, on county probation, or under post release community supervision.
6)  Families of mixed-immigration status.
7)  Persons with limited English proficiency.
 
The El Dorado County Community Health Center (EDCCHC) will receive the majority of this funding in order to carry out the Work Plan activities.  EDCCHC is a Federally Qualified Health Center (FQHC) that is well versed in providing primary healthcare services to a high volume of Medi-Cal recipients, and has the infrastructure in place to build on existing Medi-Cal O&E services and further enhance outreach to the target population of potentially Medi-Cal eligible residents countywide.  The Health and Human Services Agency (HHSA) now brings Agreement for Services 069-S1511 with EDC-CHS for the provision O&E activities in accordance with the Work Plan incorporated in the DHCS grant to the Board for approval and signature by the Chair.  
 
Reason for Recommendation
Approval of Agreement for Services 069-S1511 between the Health and Human Services Agency and El Dorado County Community Health Center will enhance HHSA's ability to provide necessary outreach services to the underserved populations targeted by this grant.  The expected result is to link these individuals to Medi-Cal benefits, thereby providing them access to necessary health care and services.  Should the Board not approve this Agreement for Services, many individuals who may be reached through O&E activities could continue to live in the community without benefits or access to necessary health care and services.
 
Clerk of the Board Follow up Actions
1)  The Clerk of the Board to obtain the Chair's signature on the two (2) originals of Agreement 069-S1511.
2)  The Clerk of the Board to return one (1) fully executed original Agreement to HHSA Contracts Unit at Briw Road for further processing.
