DEPARTMENT RECOMMENDATION
Health and Human Services Agency (HHSA) recommends the County of El Dorado Board of Supervisors approve and authorize the Chair to sign Amendment 3 to Agreement for Services 047-S1310 with New Morning Youth and Family Services, Inc.  HHSA receives funding under and Agreement with the California Department of Health Care Services for the provision of Substance Use Disorder prevention and treatment, which mandates HHSA to make substance abuse treatment services available to Medi-Cal beneficiaries through the utilization of Federal and State funds available pursuant to Title XIX and Title XXI of the Social Security Act.  The County has neither the facilities nor the staff available to meet the service requirements.  Failure to contract with community-based alcohol and drug treatment providers would result in an inability to meet the service demands of the community, as well as an inability to provide the service levels mandated by the funding agreement.  HHSA recommends approval of Amendment 3 to allow continued provision of alcohol and drug counseling and treatment services for both existing and future clients.
 
DISCUSSION / BACKGROUND
The Substance Abuse and Mental Health Services Administration (SAMHSA) is a branch of the U.S. Department of Health and Human Services, and is charged with improving the quality and availability of prevention, treatment, and rehabilitative services to reduce illness, death, disability, and cost to society resulting from substance abuse and mental illness.  SAMHSA offers many grant opportunities, including the Substance Abuse Prevention and Treatment (SAPT) Federal block grant, which is mandated by the U.S. Congress to be used to fund substance abuse and mental health services.  In California, the public system of care for the prevention and treatment of Substance Use Disorder (SUD) is overseen by the California Department of Health Care Services (DHCS) and is administered by the counties.  DHCS annually receives SAPT, Medicaid, and State Behavioral Health Realignment funding that it makes available to counties to provide these mandated substance abuse prevention and treatment services.
 
The County of El Dorado Board of Supervisors approved Agreement 284-F1511 (DHCS 14-90055) with DHCS on December 2, 2014 for the provision of SUD services during the term of July 1, 2014 through June 30, 2017 (File ID 14-0612, Item 13).  One of the missions of the HHSA is to provide an array of evidence-based services designed to prevent and reduce alcoholism and drug addiction.  HHSA's SUD programs accomplish this mission through education, prevention, treatment, and counseling services offered directly through contracts with community agencies.  
 
HHSA has contracted with New Morning Youth and Family Services, Inc., for the provision of Alcohol and Drug Program services since 2007.  On August 14, 2012, the Board of Supervisors approved Agreement for Services 047-S1310 with New Morning Youth and Family Services, Inc. for the term July 1, 2012 through June 30, 2013 and a maximum contractual obligation of $75,157 (File ID 12-0934, Item 6).  On September 10, 2013, the Board of Supervisors approved Amendment 1 to Agreement for Services 047-S1310, thereby extending the term to June 30, 2014 and increasing the maximum contractual obligation to $111,019 (File ID 12-0934, Item 24).  On March 11, 2014, the Board of Supervisors approved Amendment 2 to Agreement for Services 047-S1310, thereby extending the term to June 30, 2015 and increasing the maximum contractual obligation to $326,019 (File ID 12-0934, Item 16).  Amendment 3 to Agreement for Services 047-S1310 increases the term to June 30, 2017 and increases the maximum contractual obligation by $271,000 for a maximum obligation of $597,019.
 
Maximum contractual obligations are determined based upon historical financial analysis and taking into consideration other factors such as changes in the Drug Medi-Cal services and client Medi-Cal eligibility.
 
ALTERNATIVES
The Board of Supervisors may elect to not approve the Amendment and allocate funding from the SUD Agreement to other community-based alcohol and drug prevention, counseling, and treatment providers.

FINANCIAL IMPACT
There is no Net County Cost associated with this Agreement.  Sufficient appropriations were included in the fiscal year 2014-15 budget, and will be included in future budgets for the term of the Agreement.

1. Clerk of the Board to obtain signature of Chair on two (2) original Amendment 3 to Agreement for Services 047-S1310.
2. Clerk of the Board to return one (1) fully executed Agreement to HHSA at 3057 Briw Road.
