Fiscal Impact/Change to Net County Cost:Grant payments will be issued in three installments.  The first payment of $100,000 will be sent to the Department after signed documents are received by the Foundation.  Payments two and three ($70,000 and $30,000, respectively) will be issued upon review of interim reports sent by the Department to the Foundation.  There is no net County cost.
 
Background: Several months ago, Public Health identified a grant opportunity with the Foundation to supplement funding for its activities toward improving healthcare information sharing in El Dorado County.  The Department applied for, and the Foundation has approved, a $200,000 grant for the Department to be used in this endeavor, supporting the ACCEL Health Information Technology Program over a 15 month period, 9/1/2006 to 12/1/2007.
 
Reason for Recommendation:Funds provided under this grant will be used to develop a community Master Person Identifier (MPI), which is a necessary component to a functioning county-wide electronic health record (EHR) system that will enable the exchange and sharing of patient clinical information among area providers, including two community hospitals, the Public Health Department, Mental Health Department, the Federal Qualified Health Center, rural and tribal health clinics, private healthcare providers and ancillary service providers with the goal of improving care delivery, quality, and coordination.
 
This Agreement is retroactive to September 1, 2006 per the stipulations of the grant.  However, the Department received grant documents from the Foundation on September 5, 2006.  Public Health processed the Agreement as quickly as possible and brought it to the Board at the earliest available meeting.  The first grant payment will be issued upon receipt by the Foundation of this signed grant agreement.
 
 
Action to be taken following Board approval:Chairman to sign two (2) copies of the attached grant agreement; Chairman to sign attached Budget Transfer; Board Clerk's Office to forward executed Budget Transfer to Auditor's Office for posting; Board Clerk's office to forward one signed copy of Agreement to Public Health; Public Health to forward signed copy to the Foundation and distribute copies as appropriate.
