Fiscal Impact/Change to Net County Cost: $115,000 has been included in adopted budget and will be funded by the AHRQ grant under the ACCEL Technology Implementation Project.  There is no Net County cost.  Payment for services rendered by Consultant shall not exceed $196,000 over the entire term of the contract.
 
Background: The Public Health Department, in collaboration with the ACCEL Initiative staff, has taken the lead on the El Dorado County Safety Net Provider Project that brings health care organizations in our County together to integrate systems of care administratively, clinically, financially, and technologically with the goal to strengthen the health care delivery system directed to the poor and underserved.  Various grants have been written and received in support of this effort, including the one that supports this Agreement.  Specific goals include 1) a web-based regional health record 2) a Networked Care Connections Program to provide a medical home and improved medical services to low income/minority, uninsured and/or publicly-insured El Dorado County residents; 3) implementation of a county-wide technology infrastructure that will permit high speed connectivity between participating organizations; and 4) implementation of a system of electronic prescription writing for providers.
 
Reason for Recommendation: Healthcare Tech has been providing expertise in planning, procurement, and implementation of applications and systems used to support these goals. The work is being done in conjunction with County IT. The Department wishes to include additional components to the scope of this contract, which include selection and procurement services to acquire a county-wide Health Information Exchange (HIE) ($60,000) and the Development of an economic cost model, sustainability & business plan ($55,000).  The term of this agreement shall also be extended to June 30, 2007 to allow for the provision of these additional services.
 
Action to be taken following Board approval: Chair to sign three (3) originals of attached Amendment; 2) Board Clerk's Office to forward two (2) signed documents to Public Health; 3) Public Health to distribute as appropriate.
