*Amount of additional funding for the three-year Agreement. $12,000 is the amount of additional funding for FY 2006-07. The remaining $31,000 will be budgeted in FY 2007-08 and FY 2008-09.

Fiscal Impact/Change to Net County Cost: Total amount of funding under this three-year Agreement is increased by $43,000. There is no net County cost.

Background: The Board of Supervisors approved the original PHCA Grant Agreement at their meeting of July 18, 2006. The grant provides funding for preventive health programs serving the non-frail, older adult (55+) population, including low-income and minority older adults.

Reason for Recommendation: This amendment reflects an expansion of program services in the Scope of Work of the original Agreement and an increase in the budget to compensate the County for performing the additional services over the three-year period. The added work is related to Health Promotion, participation in the Arthritis self-help program, and the County's offering of at least four self-help programs annually.

This amendment is retroactive to October 1, 2006 due to late receipt of documents from the State.
