Human Services Department, Community Services Division, recommending Resolution authorizing the Chairman to execute Agreement MS-0708-35, Contractor/Vendor Confidentiality Statement, and Certification of Compliance CCC-307 with the California Department of Aging in an amount not to exceed $321,375, for the period July 1, 2007 through June 30, 2008 for the Multipurpose Senior Services Program; and authorizing the Director of Area Agency on Aging to administer said Agreement and execute any subsequent documents relating to same, including amendments thereto, that do not affect the dollar amount or term and required fiscal and programmatic reports, contingent upon approval of County Counsel and Risk Management.
RECOMMENDED ACTION: Adopt Resolution 152-2007.

Fiscal Impact/Change to Net County Cost: No change. The $321,375 in Federal and State funds payable by the State to El Dorado County under this Contract is included in the FY 2007-08 Proposed Budget Request.
Reason for Recommendation: Contract MS-0708-35 provides $321,375 in Federal and State funding to provide the Multipurpose Senior Services Program in El Dorado County during FY 2007-08. These services assist Medi-Cal eligible, frail, vulnerable seniors to remain in their homes and independent to the extent possible.
Action to be taken following Board approval: Board Clerk to provide Human Services, Community Services Division, with three (3) certified resolutions, three (3) copies of the full contract MS-0708-35, three (3) contract face sheets, three (3) Contractor/Vendor Statements of Confidentiality (CDA 1024) and three (3) CCC-307 Certifications signed by the Chairman.
