Background: The Mental Health Department has contracted with Maxim Healthcare Services, Inc. to provide licensed health care providers to supplement the availability of personnel for the Psychiatric Health Facility on an "as needed" basis.  The original amount of the agreement was for an amount not to exceed $10,000.00 with a term to expire April 10, 2007.  Amendment I was processed increasing the compensation by $25,000.00 for a total not to exceed amount of $35,000.00.  Amendment II was processed extending the term through April 30, 2007 (pending a new contract beginning May 1, 2007) and increasingthe compensation by $65,000.00 for a total not to exceed amount of $100,000.00.  Procurement and Contracts has prepared Amendment III to increase the compensation by $4,000.00 to cover additional invoices received for services provided within the term of the agreement.

The Purchasing Agent has coordinated the review of the consultant's qualifications to perform the needed services with the Psychiatric Health Program Manager from Placer County and the Director of Nursing from Sacramento County's Mental Health Department.   Both evaluators have confirmed that Maxim Healthcare Services is qualified and able to perform the necessary services.  Both Placer and Sacramento County also contract with this firm for similar services.

The County Purchasing Agent has reviewed the department's request to amend the contract with Maxim Healthcare Services Inc.  and concurs that the department's request is in compliance with Board of Supervisors Policy C-17, Sections 7.4.2, 7.5, and 7.10.

Reason for Recommendation:This amendment will allow the Mental Health Department to pay the final invoice for this vendor for the agreement which expired on April 30, 2007. A new agreement with this vendor is already in place.

Action to be taken following Board approval:Board Clerk's Office will forward the executed amendment to Mental Health for further processing.
