Reason for Recommendation:

On November 6, 2007, the Board of Supervisors approved the County's participation in the ACCEL (Access El Dorado) Care Pathways Program to build a collaborative case management program within El Dorado County.

Currently, the participating agencies include: Barton Healthcare System, Barton Community Clinic, Barton Memorial Hospital, Tahoe Family Physicians, Marshall Medical Center, Marshall Physicians Care Services, El Dorado County Community Health Center, El Dorado County Mental Health Department, El Dorado County Public Health Department, and Shingle Springs Tribal Health Clinic.

As a covered entity, El Dorado County must comply with Code of Federal Regulations Title 45 Part 164.520 which states an entity must promptly revise and distribute its notice of privacy practices whenever there is a material change to the uses or disclosures, the individual's rights, the covered entity's legal duties, or other privacy practices stated in the notice. Therefore, individuals will be informed of ACCEL and their rights by revising the Notice of Privacy Practices. The Notice will be revised by including the following language:

"We may also share your health information with ACCEL (Access El Dorado). ACCEL is a health program that allows community health providers to work together for improved care in El Dorado County. Health care providers involved in your care can share information th...
