Background:
In November 2004, California voters passed Proposition 63, now known as the Mental Health Services Act (MHSA).  A Community Services and Supports (CSS) plan was developed to determine how MHSA financing would be used in the County.  CSS plan development included active participation of key organizations and individuals from throughout El Dorado County, including community leaders and non-profit organizations providing charitable and public benefit activities for El Dorado County residents.  In January 2006, the Board of Supervisors (BOS) approved the three-year CSS plan for submission to the California Department of Mental Health.  El Dorado County's CSS plan was formally approved by the State on April 28, 2006 and the County presently receives MHSA funding through Agreement number #644-S0811 with California DMH.    
 
One of the programs identified in the County's CSS plan is a Wraparound program for children and transitional age youth.  Wraparound is an intensive, individualized case management process for children and adolescents with serious or complex psycho-social problems and/or emotional disturbance.  During the wraparound process a multi-disciplinary team (including family members) relevant to the specific issues affecting a child or adolescent work to collaboratively develop an individualized plan.  The Wraparound plan places a premium on deferring to a client and family member's own perceptions of needs and goals.  At the same time, plans are generally aimed at improving academic success; reducing recidivism or preventing entry into the juvenile justice system; and reducing long-term dependency on welfare systems.  
 
Reason for Recommendation:
Tahoe Youth and Family Services (TYFS) is a public benefit organization organized for charitable purposes; namely, to promote the healthy development of the children, youth, and families of the Lake Tahoe community through counseling, educational, and support programs.   TYFS is one of various key organizations in South Lake Tahoe on the front lines working to contain increase in homelessness, alcohol and drug dependency, and mental illness in El Dorado County communities, and to address and reduce the prevalence of these issues.  Preventing and reducing the incidence of suicide, teen pregnancy, gang involvement, incarceration and runaways are among the key priorities of this charitable organization.  
 
The Wraparound program is one of the means by which TYFS pursues seeks to address these issues while at the same assisting the County by serving as part of the system of mental health care delivery for county residents.  The Department has a contract with TYFS that took effect October 26, 2006 and was subsequently amended on February 27, 2007 (Agreement #181-S0711) to implement the Wraparound program.  Under the present contract, TYFS is paid at hourly rates based on the staff time provided, with different rates applicable to the various types of staff that provide services.  This compensation method has proven infeasible and the MHD believes that the County, the vendor, and the clients will be better served with a new contract that includes a modified scope of services and a more feasible monthly compensation structure.  
  
For these reasons, the Department recommends that the Board terminate the existing agreement and approve a new agreement for the period of May 1, 2008 through June 30, 2009.  
 
Action to be taken following Board approval:  Board Clerk's Office will forward signed termination letter and the new executed agreement to Mental Health for further processing.
