Public Health Department recommending Chairman be authorized to sign retroactive Agreement DHA/CS-ELD-01-08 with Sacramento County Department of Human Assistance for the term January 1, 2008 through December 31, 2008 for Housing Opportunities for Persons with AIDS (HOPWA) funding in 2008; recommending Resolution authorizing submission of claims and other financial documentation for said funding; and Director of said Department be authorized to sign the Certificate Regarding Lobbying and Contractor Certification of Compliance form.
RECOMMENDED ACTION: Adopt Resolution 06-2008.
FUNDING: Grant funding through Sacramento County.

Fiscal Impact/Change to Net County Cost: County will receive $33,908 from Sacramento County to support its HOPWA activities in 2008. There is no net County cost.
Background: El Dorado and other local rural counties obtain their HOPWA and Ryan White C.A.R.E. Act funding through Sacramento County. Other AIDS Programs are funded directly by the State under an AIDS Master Grant, and include grants for 1) Education & Prevention, 2) Evaluating Local Interventions, 3) HIV Counseling and Testing, and 4) a Drug Assistance Program.
The HOPWA Program provides emergency housing assistance for El Dorado County residents and their families living with HIV disease. Short term assistance and supportive services are provided to qualified persons for rent, mortgage, and utilities to prevent homelessness.
Reason for Recommendation: This new contract with Sacramento County for HOPWA services is for the term 01/01/08 through 12/31/08. The attached documents are part of the complete contract package requested by Sacramento County. El Dorado County HOPWA services are subcontracted out to Sierra Foothills AIDS Foundation, a community based agency specializing in services to individuals and families afflicted with AIDS. Sierra Foothills AIDS Foundation maintains all appropriate credentialing and has succesfully passed annual site reviews. This agreement is retroactive to January 1, 2008 due to late receipt of documents from Sacramento County. The Department processed the agreement as quickly as possible for Board review.
Action to be taken following Board approval:
1) Board Chairman to sign two (2) copies of the attached Resolution and four (4) copies of the attached agreement where indicated;
2) Board Clerk to return one copy of signed resolution and four (4) signed copies of agreement to Department;
3) Director to sign Contractor Certification of Compliance Form and Certificate Regarding Lobbying (Enclosure I);
4) Department to forward to Sacramento County.;
5) Department to return fully executed copy of agreement to Board Clerk upon receipt from Sacramento County.
