Fiscal Impact/Change to Net County Cost: Amendment extends term of agreement only. No additional funds are associated. There is no net County cost.

Background: The Board approved this $200,000 grant from the Blue Shield of California Foundation at their meeting on November 14, 2006. It has supported the ACCEL Health Information Technology Program over a 15 month period, 9/1/2006 to 12/1/2007. Funds provided under this grant have been used to develop a community Master Person Identifier (MPI), which is a necessary component to a functioning county-wide electronic health record (EHR) system, enabling the exchange and sharing of patient clinical information among area providers, including two community hospitals, the Public Health Department, Mental Health Department, the Federal Qualified Health Center, rural and tribal health clinics, private healthcare providers and ancillary service providers with the goal of improving care delivery, quality, and coordination.

Reason for Recommendation: Despite rigorous efforts by the Department and its partners to complete the work supported by this funding, the total amount of the grant was not spent by the end of the grant term. The Foundation, therefore, agreed to extend the term of the grant agreement by five months so that the remaining work could be completed.

This amendment is retroactive to the term of the grant. The Department received documents from the Foundation on December 7, 2007, six days after the grant agreement had expired. Public Health processed the amendment as quickly as possible.
