Fiscal Impact/Change to Net County Cost:  County shall receive $49,998 over a seven month period, February 1 through August 31, 2008.  This is a new grant award.  There is no net County cost.
 
Background: The El Dorado County Public Health Department, on behalf of ACCEL (Access El Dorado - a coalition of safety net providers) has sought funds to assess specialty care shortages and develop an implementation plan for innovative delivery models to improve access to healthcare for County residents.  The California HealthCare Foundation recently has awarded a grant to the Department to support planning activities, specifically titled the ACCEL Specialty Access Project, over a seven month period.
 
Reason for Recommendation: The Public Health Department will serve as lead agency and fiscal agent for the Project.  The project will be developed as part of the ACCEL (Access El Dorado) program, a collaborative effort of county-wide public and private health organizations, 2 hospitals (Marshall and Barton), clinics (FQHC, rural, tribal), private practicing physicians, El Dorado County Public Health and Mental Health Departments to improve access to health care for the safety net population of the County.  
 
The goals and objectives of the Project:
 
1) Conduct a comprehensive specialty care needs assessment in the County, which shall include: a) determining the current level of specialty care accessible to Medi-Cal and other under and uninsured patients in both regions (West Slope and South Lake Tahoe) of El Dorado County, b) identifying the need for specialty care among primary care and safety net providers, and c) completing a baseline assessment of the need for such specialty care in El Dorado County;
 
2) Engage broader participation among county healthcare providers to review and evaluate alternative approaches that will increase cost effective access to specialty care throughout El Dorado County; and
 
3) Develop an implementation plan to pursue alternative approaches to increase specialty care for at least two separate types of services, while formalizing project leadership within ACCEL and analyzing alternative approaches to expand specialty care access and resources necessary for an expanded primary care capacity and improved specialty referrals.  
 
This grant agreement is retroactive to February 1, 2008 due late receipt of grant documents from the Foundation.  Public Health received those documents on January 14 and processed the agreement as soon as possible.  A counter signature by the Foundation is not required for this agreement - it is considered binding and effective upon signature by the Chairman.
 
Action to be taken following Board approval:  Chairman to sign two (2) copies of the attached grant agreement; Board Clerks Office to return one copy to Public Health; Public Health to forward one original to the Foundation; Chairman to sign the attached Budget Transfer; Board Clerks Office to forward signed Budget Transfer to Auditors Office for posting.
