Fiscal Impact/Change to Net County Cost:  No change to net County cost.  Funds are included in the Department's budget for FY 2008-09.   Funding for future years will be included in the Department's budget request.  The original Agreement provided that the County would pay Barton a Not-to-Exceed amount of $667,100 through the term of the contract ending September 29, 2008.  Amendment I increased the Not-to-Exceed to $687,100.  This Amendment II specifies that any unspent funds remaining on September 29, 2008 (minus $10,000 being allocated for other grant activities), will be made available for Barton to complete work on this project through September 30, 2009.
 
Background:    The Health Services Department along with other major partners in the Access El Dorado (ACCEL) initiative, have been working to bring health care organizations in the County together, integrating systems of care administratively, clinically, financially and technologically.  ACCEL's goal is to strengthen the healthcare delivery system, with special focus directed to the poor and underserved in El Dorado County.
 
The primary funding grant from AHRQ was awarded to Marshall Medical Center and funding transferred to County via a separate agreement with Marshall Medical Center.   AHRQ has awarded a no-cost extension through September 29, 2009 to allow completion of the project and this Amendment II to the Agreement with Barton Healthcare will allow Barton to complete work on the project.
 
Reason for Recommendation:  Approval of the Amendment II to Agreement 566-PHD0207 will allow Barton Healthcare and County to complete work under the AHRQ grant extension.
 
Action to be taken following Board approval:
1.      Chairman to sign three (3) Amendments II to Agreement for Services 566-PHD0207.
2.      Board Clerk's office to return two (2) fully signed Amendments II to Agreement for Services       566-PHD0207 to the Health Services Department.
3.      Health Services Department to distribute documents as appropriate.
