Title: Health and Human Services Agency, Public Health Division, recommending the Board consider the following: 1) Authorize the Chair to sign retroactive Agreement for Services 005-F1411 with El Dorado Union High School District (EDUHSD) thereby allowing the County to provide on-site Public Health Nurse services to EDUHSD for the term of July 1, 2013 through June 30, 2016 and a total reimbursement to the County in the amount of $90,000; and 2) Authorize the Chief Administrative Officer or designee to execute further documents relating to the above noted Agreement, including amendments thereto, contingent upon approval by County Counsel and Risk Management, which do not affect the maximum dollar amount or term of the Agreement. Fiscal Impact/Change to Net County Cost
There is no Net County Cost associated with this funding Agreement.  The anticipated funding is included in the Fiscal Year ("FY") 2013-2014 budget.
 
Retroactive Term
Agreement is retroactive due to necessary revisions to the Agreement and delays in receiving appropriate insurance documentation from El Dorado Union High School District.
 
Background
Since 1996, and initially due to a State sponsored Healthy Start grant, the County of El Dorado has been an active participant in collaboration with the El Dorado Union High School District ("EDUHSD") in providing preventive health care intervention and education to at-risk students and their families through the provision of a centralized and campus-based Public Health Nurse ("PHN").  The PHN provides expert assessment and case management for those students and their families that are at highest risk for complex health, psychosocial, and academic challenges resulting in students being able to access direct referrals to a full scope of community-based resources.
 
Reason for Recommendation
Approval of Agreement for Services 005-F1411 will allow for the continuing and successful collaboration between the County and EDUHSD in the provision of preventive health care intervention and education to at-risk students and their families in the County of El Dorado.
 
Action(s) to be taken following Board approval
1.      Chair to sign two original Agreement for Services 005-F1411;
2.      Board Clerk to return one signed original to HHSA, Contracts Unit, at 3057 Briw Road, Suite A;
3.      HHSA to submit the originally signed document to the El Dorado Union High School District; and
4.      As applicable, the Chief Administrative Officer or their designee to execute further documents relating to Agreement for Services 005-F1411, including amendments thereto, contingent upon approval by County Counsel and Risk Management, which do not affect the maximum dollar amount or term of the Agreement.
