Fiscal Impact/Change to Net County Cost:   There is no Net County Cost associated with this funding Agreement.  
 
Background:   The original grant agreement with First 5 El Dorado Children and Families Commission ("First 5") to assist families in the County of El Dorado with access to health insurance and health services for children ages 0-5 years occurred in fiscal year (FY) 2004-05. Grant funding has been provided each subsequent year to allow for continuation of these services. In FY 2010-11, the grant agreement incorporated funding for access to health insurance and health care services, as well as access to oral health care for children ages 0-5 years.
 
The grant agreement for FY 2011-12 again incorporates funding for access to health insurance and health care services, as well as access to oral health care for children ages 0-5 years. The objectives for this grant agreement 054-105-P-R2011 include:
 
1.      Identify children ages 0-5 years with Medi-Cal and assist them (and their eligible siblings through 17 years of age) to establish a dental home on the El Dorado Children's Dental Van.
2.      Identify children without health and/or dental insurance and assist them to determine eligibility and obtain insurance, establish a medical and a dental home, and utilize their medical and dental homes for well child exams, checkups, and non-emergency health care.
3.      Integrate Children's Health Initiative strategies within Commission Initiatives and the Early Childhood System.
 
Compensation:
County to be paid ten percent (10%) of contract total upon signing and approval of the contract, and monthly invoices to be paid according to terms outlined in the contract.
 
Termination:
Either party may terminate this contract for any reason by giving thirty (30) calendar days written notice to the other party. Notice of termination shall be by written notice to the other party and be sent by registered mail.
 
Retroactive Term:
The proposed contract was received by the Department June 29, 2011. Staff immediately processed agreement for County Counsel review and prepared for the next available board date.
Reason for Recommendation:   The services provided through this grant agreement are essential to ensure healthy children within our community and are consistent with the Health Access priority in the First 5 El Dorado Children and Families Commission's strategic plan.
Action to be taken following Board approval:
1.      Chair to sign three (3) originals of Agreement 054-105-P-R2011.
2.      Board Clerk's Office to return all signed original Agreements to Department.
3.      Department to return all originals to First 5 for final signatures.
4.      Upon receipt of fully executed Agreements, Department to return one (1) original to Board Clerk's Office.
