Health and Human Services Agency, Mental Health Division, recommending the Board authorize the Chair to sign Amendment I to Agreement 583-S1211 with Denise Thompson (dba The Denise Thompson Home) for the provision of residential treatment services for adults with serious mental illness, amending Article I "Scope of Services" to add language to ensure ongoing consistency with the El Dorado County Mental Health Services Act Plan regarding Full Service Partnership services. There is no change to the existing term of June 26, 2012 through June 30, 2013 and no change to the current not-to-exceed compensation amount of $375,000. (Refer 6/26/12, Item 18)

Compensation for Services
Payments are made to Contractor based on monthly invoicing and at specified rates as detailed in the Agreement for actual services performed and documented per contract requirements.
Termination
This Agreement includes the County's standard Fiscal Considerations termination language as well as a provision for the County to terminate the Agreement without cause upon seven (7) calendar day's written notice.
