DEPARTMENT RECOMMENDATION: 

The Health and Human Services Agency (HHSA) is recommending the Board approve, in concept, HHSA to explore options related to establishing a Mental Health Rehabilitation Center (MHRC) located in the County of El Dorado.  HHSA would thereafter return to the Board to report the outcome of this research.

 

DISCUSSION / BACKGROUND:

HHSA has contracted with multiple vendors for several years to ensure availability of services in mental health rehabilitation center(s).  A mental health rehabilitation center means a 24-hour program, licensed by the California Department of Health Care Services, which provides intensive support and rehabilitation services designed to assist persons, 18 years or older, with mental disorders who would have been placed in a state hospital or another mental health facility to develop the skills to become self-sufficient and capable of increasing levels of independent functioning (Title 9, California Code of Regulations [CCR] Subchapter 3.5, Section 782.34).

 

Current contracts include:

1) California Psychiatric Transitions, ninety-eight (98) beds located in Delhi, CA, for the term April 1, 2016 through March 31, 2018, with a maximum obligation of $1,500,000.  During the term of this Agreement, HHSA has admitted 11 clients, for 2,789 bed days, with a total cost of $1,210,500.

 

2) Crestwood Behavioral Health, Inc., three hundred thirty-seven (337) beds located in various facilities throughout California, for the term July 1, 2016 through June 30, 2019, with a maximum obligation of $3,000,000.  During the term of this Agreement, HHSA has admitted 20 clients, for 3,288 bed days, with a total cost of $751,185.

 

3) Mental Health Management I, Inc, dba Canyon Manor, eighty-nine (89) beds located in Novato, CA,  for the term July 1, 2016 through June 30, 2019 with a maximum obligation of $337,500.  During the term of this Agreement, HHSA has admitted 3 clients, for 673 bed days, for a total cost of $203,330.

 

4) Willow Glen - Sequoia facility, sixteen (16) beds located in Yuba City, CA, for the term July 1, 2013 through December 31, 2018, with a maximum obligation of $1,050,000.  During the term of this Agreement, HHSA has admitted 7 clients,  for 793 bed days, with a total cost of $203,619.

 

Establishing an MHRC in the County would allow HHSA to transfer clients currently admitted to contracted MHRCs back to the County, thereby allowing the clients the availability of natural support systems within their own community, availability of family members to assist in their ongoing recovery, and continuity of care and treatment upon transition back into the community. 

 

Additionally, based on historical usage of this level of care, HHSA is recommending the Board authorize HHSA to explore up to a forty (40) bed MHRC facility.  Such a facility would need to comply with I-3 (institutional occupancy classification) code requirements, including semi-private and private rooms for flexibility.  A typical forty (40) bed facility would include two, twenty-bed units, including a common day room, group room, toilet/shower rooms and staff support area.

 

ALTERNATIVES:

Should the Board decline to authorize HHSA to explore options to establish an MHRC in the County of El Dorado, HHSA would continue using contracted facilities to ensure availability of mental health rehabilitation center services to the community.
